EMS/Ambulance records requests need to be in writing and either faxed to 281-852-2630, or mailed to Station 1 7642 Farmingham Rd, Humble, TX 77346 ATTN: EMS Records/EMS Chief. The cost of an EMS/Ambulance record is $35.00. The fee is waived for patients requesting their own records, and for the District Attorney's office. In accordance with Federal and State Privacy Laws we may refuse to disclose patient medical records. To obtain patient medical reports, you must comply with the laws governing protection of these reports (HIPAA). Once you are finished with these reports you must return them, properly store them, or completely destroy them.
